2023 DGF Summer Recreation Cancellation Request Form
1. Full refunds are given when Community Education cancels a class.  
2. If you need to cancel or withdraw from a program, please submit this Cancellation Request Form.  Check all the activities you are needing to cancel. There is a $10 cancellation fee per activity.
3. Refund requests must be received at least five business days before the start of the class and will include a $10 cancellation fee per activity.
4. No refunds will be issued after the first day of class.
5. Please allow 2-3 weeks for the processing of a refund. Refund will be mailed to your address on file or via payment method used.
