By signing this consent form, I the undersigned participant, fully accept the following terms and conditions:

1. Participant takes part in the „Individual and team level conditions and organizational needs for successful problem solving / project implementation workshop” organized by the EIT Health Partner: Institute of Transdisciplinary Discoveries (Medical School, University of Pécs) (hereinafter: RIS Hub) under the Regional Innovation Scheme activities implemented in partnership with EIT Health InnoStars (hereinafter InnoStars);

2. No contract of employment, nor services contract, nor subcontract for work is concluded between the participant and RIS hub or EIT Health InnoStars e.V or EIT Health e.V for the activity mentioned under section 1;

3. The participant gives consent to EIT Health InnoStars, EIT Health, EIT and the RIS Hub to process his/her personal data for reporting purposes and in cases if it is necessary to comply with their audit/review obligations towards the relevant bodies.
