DO NOT USE THIS FORM FOR THE FOLLOWING:

1. If you are a parent of/a current student and have moved and are transferring to another school, please contact Alicia Wright/SLOHS Registrar via email (awright@slcusd.org) or phone (805-596-4040 ext 31941).

2. Common Application official transcript requests will be completed by your counselor as part of the initial and/or mid-year report. Please email or speak with your counselor to request transcript.
