Please note that all bracelets start at 15$ For 1 allergy or medical condition. And that additional allergies or medical conditions can be added for 5$ each. (up to a maximum of 4 per Item)
If paying online thru the www.raynbands.com website, please note all price include the 5$ shipping fee.

Once this form is received, Please ensure you send payment by E-Transfer to raynbands@gmail.com if you have not already paid thru PayPal via the website.

Please note, Only Orders for which payment have been received will be processed and confirmed via email confirmation.
