Draw days will be on the 1st or the 15th of every month.

Your club membership cannot be used by or transferred to any other person.

Notice of cancellation must be submitted a minimum of 3 days prior to the next payment due date (1st or 15th of the month).

If you cancel your membership, you will forfeit your club membership activation fee of $39 and upon re-enrollment you may be required to pay the club membership activation fee again.

You may choose to freeze/hold your membership by calling, walking in, or online at least 3 days prior to your draw date to freeze/hold your membership. There will be a $5.86 processing fee for each month the membership is frozen/on hold, this fee is nonrefundable.

Hawaiian Tan reserves the right to terminate any membership for non-payment of dues.

By accepting Yes below, I authorize Hawaiian Tan to withdraw my monthly membership dues from the specified account in accordance with the terms of this agreement.
