John Henderson PAC - Request for Hot Lunch refund
This form is to be used if you are requesting a refund for Hot Lunch orders made for April through June 2020.
Form must be completed in full and submitted before 11:59pm on Tuesday July 7, 2020 in order to be considered.
All requests will be compared to our original order forms. Once validated, a member of PAC will contact you to make arrangements for your refund.
If you are requesting for multiple students, please complete a request for each one individually
