BNHS DAZZLERS JUNIOR DANCE CLINIC REGISTRATION - SEPTEMBER 25, 2021
REGISTRATION FEE  $40
WHO:  Anyone grades K though 11th grade  (dancers are grouped according to age group)

WHAT YOU WILL RECEIVE:
-Dance instruction with the Byron Nelson Bobcat Dazzlers Dance Team
-Opportunity to perform at the Dazzlers Annual Craft Show- Saturday, November 20th (More info to come!) Entry to craft show is free.
-Dazzler Jr. Clinic T-Shirt (Sizes limited if registered after 9/19)
-Opportunity to perform with the Dazzlers for pregame at the BNHS varsity football game- Friday, October 1st including 2 tickets to the game for spectators. More info below.


WHERE: BNHS Main Gym
*Drop off and check in will be in front of the main gym* (Dazzlers will be there to greet you and show you where to go)

TIMES:  
8:45 AM - 9:00 AM - CHECK IN
9:00 AM - 11:30 AM - CLINIC
11:30 AM - PARENT SHOW in the BNHS Main Gym

We will NOT be accepting mailed in/dropped off cash & checks, you MUST pay online prior to the event through the methods below.

WHAT TO WEAR FOR CLINIC:
Jr. Dazzlers will need to wear dance or athletic clothing that they can move around comfortably in. Tennis shoes or jazz shoes will work great. Hair should be pulled back out of face.

WHAT TO BRING FOR CLINIC:
-Breakfast should be eaten beforehand
-Water bottles that can be refilled at the water fountain
-Snacks that will be easy to eat on a short break

BNHS Varsity Football GAME INFO:
Friday, October 1st, 2021
Northwest ISD Stadium
6pm meet time for pregame performance. Meet by the helmet at the north end zone. Dazzlers will meet you by the field. Parents will NOT be allowed on the field.
7pm Game Time
*Participants will receive 3 tickets total (1 for participant, 2 for guardians)

Dazzlers Annual Craft Show:
November 20th, 9:00am performance, arrive at 8:30am

In the event that I cannot be reached to make arrangements for emergency medical or dental attention, I authorize the team director, or persons in charge, to take my child to the nearest available hospital or health clinic.
ASSUMPTION OF RISK/RELEASE OF LIABILITY *
It is understood that Northwest ISD does not provide medical insurance covering injuries of any nature incurred during the Dance Clinic or Clinic Performance. The undersigned hereby releases Northwest ISD from any and all claims, demands, and causes of action whatsoever in any way growing out of, or resulting from participation in this clinic. All participants should be covered by their own insurance policies.
WEBSITE/MEDIA RELEASE *
I grant permission to the Bobcat Dazzlers Drill Team to publish pictures of my child participating in Dazzlers Drill Team activities on the Northwest ISD website. I understand that the organization will not publish first and last names or any additional information of my child. I understand that group pictures where individuals are not readily identifiable do not need this permission form.
