Forten Scholars Early College Program - Family Form
We are happy to welcome your student as a member of the newest cohort of Forten Scholars! This will be an excellent opportunity for your child to jumpstart their college education, and we look forward to sharing the year with you and your scholar.

This program is entirely free including cost of college tuition, books, and transportation.

Upon completion of the three-year program, there is a potential to earn up to 27 college course credits that will also count towards high school credits. All credits earned with a C or better can be transferred to any college, saving families thousands of dollars.

Student Expectations: (1.) I will respect those around me (faculty, staff, classmates and myself) and behave in a mature and appropriate manner.   (2.) I will maintain a positive attitude.  (3.) I will come to class on time with all the required materials and assignments.  (4.) I will NOT use any electronic devices during class.  (5.) I will maintain any textbooks provided to me in good condition and return them at the end of class.  (6.) I will let my teacher(s) know if I need assistance with class assignments or activities so we can assist with your academic success.  (7.) I will abide by all of the policies indicated in the Salem High School and Salem State University Code of Student conduct. If a student disregards any of these policies they are subject to disciplinary sanctions that could include a failing grade for an assignment, the course, or dismissal from the Forten Scholars Early College Program. As a Parent/Guardian of a Forten Scholar: I grant permission to reproduce and distribute videos, films, photographs, and sound recordings of my child for the use of documenting and promoting the Forten Scholars Early College Program. *
My signature indicates that I agree to comply with the Forten Scholars Early College Program expectations for my child listed above. If my child doesn't meet their responsibilities, I will be asked to attend a meeting with my child and their teachers to discuss changes that need to be made. If after this meeting, my child is still not meeting expectations as indicated above, they may be removed from the program. PLEASE SIGN BELOW. *
