I, the undersigned, am a parent or legal guardian having custody of (name entered above) a minor pupil who attends Newtown High School. I hereby give permission to said son, daughter or ward to travel by private car to practices or contests to be played at local ‘HOME’ sites. I understand that Newtown School District does not purchase, or have, medical/dental/hospitalization insurance to cover injuries or loss of life to pupils or to indemnify parent for expenses in connection therewith and that such insurance, if desired must be purchased by the parent or guardian. I understand that all persons traveling in private cars shall be deemed to have waived all claims against the District to injury, accident, illness or death occurring during the transportation in a private car.
