Myriad Health + Fitness DEI Survey
Unlike engagement surveys we have shared in the past, the Diversity, Equity and Inclusion Survey is unique in that it is centered around researched constructs of Inclusion, such as fairness, belonging, and voice. The purpose of this survey is for us to determine our current baseline with DEI and be able to ensure we are using data to measure progress moving forward. The data we receive will be used to make long-term decisions. This survey asks important, though sensitive, questions about demographics like gender identity, race/ethnicity, and sexual orientation.

It is not required to fill in the 'Please explain why you provided the given response' but if you want to explain a situation, share, or offer something your response would be valuable for our data collection efforts. All information collected in this survey is confidential and any/all identifying information will be only be viewable to our leadership team and DEI committee.


For this survey’s purposes we define the following words as:

Safe - An environment in which a person or category of people can feel confident that they will not be exposed to discrimination, criticism, harassment or any other emotional or physical harm. We are not referring to safety as it relates to movements and/or injuries in this context.

Diversity - refers to all aspects of human difference, social identities, and social group differences, including but not limited to race, ethnicity, creed, color, sex, gender, gender identity, sexual identity, socio-economic status, language, culture, national origin, religion/spirituality, age, (dis)ability, and military/veteran status, political perspective, and associational preferences.

Equity - refers to fair and just practices and policies that ensure all community members can thrive. Equity is different from equality; being equitable means acknowledging and addressing structural inequalities - historic and current - that advantage some and disadvantage others. Equality implies treating everyone as if their experiences are the same. Equal treatment results in equity only if everyone starts with equal access to opportunities.

Inclusion - Refers to the extent that all our members feel comfortable, respected, have a sense of belonging, and are able to participate to achieve their full potential. While diversity is essential, it is not sufficient. A sustained practice of creating an inclusive environment is necessary for success.
