Esperanza Health Centers in Chicago, IL is pleased to announce that it is accepting applications for its 2024 - 2025 class into the Advanced Practice Provider Post-Graduate Primary Care Fellowship. The class of 2024 - 2025 will begin September 2024.

A key objective of our training program is to reduce health inequities and to provide culturally responsive care to the patient population on the Southwest side of Chicago. A major piece of this plan is to prioritize training Advanced Practice Providers whose background and demographics match the patient population that we serve.
Application Form Requirements:
- All applicants are required to fill out this application and submit the required supporting material
- We accept applications from Family Nurse Practitioners, Adult-Gerontological Nurse Practitioners, and Physician Assistants.
- Medical Spanish language proficiency is required by the start of the program
- Three professional references
- Three essays
Upon receipt and review of your application if you are selected for an interview, we will request the following information:
- CV with work and educational history
- Copy of Illinois licenses
- Copy of licenses from other states
- Federal DEA license
- Board certification or evidence of eligibility for certification
- Professional diplomas and official transcripts for RN, NP, and/or PA school
- Three letters of recommendation.
- If applicable, non U.S. residents must provide a copy of their permanent resident card, VISA, or proof of eligibility to work in the U.S.
