Booking form for the Summer activities at All Saints Primary school organised by Sport2day. Please complete one form for each child you would like to attend. *Places are limited on each day and allocated on a first come first served basis* You will receive confirmation sent to the email you provide below and we will contact you should there be any problem with your booking.

We occasionally take promotional photos/videos for marketing purposes. If you DO NOT wish your child to be included, please indicate here

Your child will need the following items with them for each day. A packed lunch, small snack, refillable water bottle, Hat/sunscreen or coat/jumper depending on weather and clothes/footwear they are happy to run around in. Please tick the box to indicate they will bring them along on the day. We sign in and out of the back gate (next to ace base) at 9am and 3pm. *
