Acknowledgement of Risk / Informed Consent (minor)

1. I have reviewed the description of the LYRICAL DANCE Program of the Tiger Hills Recreation District and feel that I have sufficiently informed myself about the nature of the program and the activities involved.

2. Recreational activity programs, such as LYRICAL DANCE, involve certain elements of risk. Injuries may occur while participating in these activities. Examples of the types of injury which may result from participation include:

· Physical exertion and rapid movements which may cause injury to bones, joints, ligaments, muscles.

· Impact or contact with other participants, instructors, stationary objects or equipment used in connection with the program.

· Dangers arising from facility and equipment use, or participants of varying skill levels.

· Contraction of illness or contagious disease.

· Loss of or damage to personal property.

3. The risk of sustaining injuries or illness results from the nature of the activity and can occur without fault of the participant, or the Tiger Hills Recreation District, it's employees/agents or the facility where the activity is taking place. By choosing to have your child(ren) take part in these activities, you are accepting the risk that your child(ren) may be injured or become ill and bear responsibility for any injury or illness that may occur. The chance of injury or illness occurring can be reduced by carefully following instructions at all times while engaged in the activity.

4. Tiger Hills Recreation District does not provide accident, disability or medical expense insurance on behalf of the children participating in this activity.
