This is an in-person worship service with Holy Communion. We will continue to follow local guidelines on meeting safely so dates, times, and number limits could change at any time. This is a one time, one date, one service sign up. Each household (individual or family of 6) need only complete this form once for the whole family. We appreciate your patience and kindness.

Out of love and concern for others, by attending, you agree to...

1) Follow social distancing in sanctuary seating guidelines, entrance and exit people flow, and as directed from ushers to ensure proper distancing.

2) Wear a mask (for other's benefit) during the duration of your time on campus.

3) Maintain social distance with others not in your household.

If you are in the "at risk" category, do not wish to follow these guidelines, or feel the slightest bit ill, PLEASE, refrain from completing this form, STAY HOME, and DO join us ONLINE for worship and/or consider making an appointment for communion by calling our church office.
