Slope Fitness Daycare Registration
**This form must be filled out in advance the day before each time you want to bring your child!**

Daycare is open from 9AM - 2PM everyday. Maximum is 2 hours.

Daycare is $5 per child per hour.

No more than 8 children allowed in the daycare room at one time.

Children must be a year and older.

Payment for daycare can be made through your credit card on file, cash or Venmo.

Members are not allowed to leave the gym while their child is in daycare.
