MPJH Student Device Request
This form is to request a Chromebook for your student to checkout for school work.

Please note that if you have any pending charges or did not turn in your device from previous years, you will not be eligible to check out another device until the device has been paid for or returned to its home campus.

Please be patient as the librarian processes these requests. They will be processed in the order received.

Este formulario es para solicitar un Chromebook para que su estudiante lo preste para el trabajo escolar.

Tenga en cuenta que si tiene algún cargo pendiente o no entregó su dispositivo de años anteriores, no será elegible para retirar otro dispositivo hasta que haya pagado el dispositivo o lo haya devuelto a su campus de origen.

Mrs. Lewis or Mrs. Baros (from the MPJH Library) will let the student know when the computer is ready for pick up. Student will need to get the computer before the end of the day from the Library.

La Sra. Lewis o la Sra. Baros (de la biblioteca MPJH) le informarán al estudiante cuando la computadora esté lista para ser recogida. El estudiante deberá recoger la computadora en la biblioteca antes del final del día.

Parent/guardian will receive an email from Mrs. Lewis or Mrs. Baros when the device is ready for pick-up. Parent will need to come to the front desk during school hours, let front desk staff know they are there to pick up a computer for their student, and it will be delivered to them there.

Los padres/tutores recibirán un correo electrónico de la Sra. Lewis o la Sra. Baros cuando el dispositivo esté listo para ser recogido. Los padres deberán venir a la recepción durante el horario escolar, informar al personal de recepción que están allí para recoger una computadora para su estudiante y se la entregarán allí.
