19/20 Student Matinee Registration Form
Use this form to make your Student Matinee Group reservation for Cinderella, and/or MasterWorks! Student Matinee performances are not open to the public, and physical tickets are not exchanged. These performances bring school students (K-12) to the Ballet at a discounted rate.

All Student Matinee Packets, including performance day logistics and study guides, will be emailed upon the completion of your registration or prior to the performance. *Student Matinee performances may be abbreviated in order to facilitate the timely return of your students, traveling from across the State. Run time is included in the welcome packet.

MASTER/WORKS - Friday, April 24, 10:30 AM - $5 Student / $10 Adult

*In order to complete the seating assignments and for security purposes, BA cannot update the reservations after the reservation deadline. If a reservation change is approved, a reservation change fee may apply.
**An electronic invoice with associated late fees will apply following the payment deadline, in order to maintain your reservation.

Which Student Matinee Series Performance will you be attending? (*if you would like to attend more than one performance, there will be an additional section for another performance reservation in section 2). *

Number of Adult Tickets ($10) *Public & Private Schools: Adult chaperone tickets (educators or parent volunteers) may not be the same number, or exceed, the number of Student Tickets. All Students must be accompanied by at least 1 chaperone per 50 students, and there may not be more than 1 chaperone per 5 students. Ballet Arkansas has the right to request additional information for adult chaperones, and when adult numbers exceed 10 adults, Ballet Arkansas will require further information. Tickets above the ratio will be charged at full ticket value. *Homeschool educators: Adult chaperones tickets must not exceed student tickets. These tickets are reserved for educators and students, currently enrolled in K-12 for grant and security purposes. *
I recognize that there are final reservation deadlines and payment deadlines, as listed at the top of this form and on my confirmation letter. Reservation change fees (after the deadline) and late payment fees will apply. I understand that additional seats may not be available, and seats are first come first serve. I understand that Ballet Arkansas will contact me to approve my reservation. If I cancel more than 25 tickets, I will be charged a cancellation fee of $50.00 or more. *
I recognize that payments will NOT be accepted at the door, and additional seats will not be added at the door. *
