Thank you in advance for considering DJH Capital Management for your financial planning and advice needs.  

The following questionnaire is designed to collect important information to help us prepare for your complimentary discovery session.

IMPORTANT NOTES:

1. To avoid wasting valuable time, you need to be aware that unfinished/incomplete questionnaires will result in meeting cancellation.  PLEASE SPEND THE TIME TO COMPLETE THE FORM BELOW.

3. Should you require a comprehensive financial plan, our planning fees average $3,500 - 4,500. This is a one-time fee paid when you decide to move forward with our services.  If you don't require that, we are willing to engage with you on an hourly consulting or "project-basis".

4. We prefer you have 60-75 minutes set aside for the discovery session and be in front of a computer for the video meeting.  If this will be a problem please let us know beforehand.
