Thank you for choosing Lakes International Language Academy! Please complete this application if you are interested in enrolling your student in Kinder Prep Preschool for the 2021-2022 school year. A non-refundable registration fee of $50.00 is required before your application is considered complete. A confirmation email with a payment link will be sent within 72 hours of submission of this application. Tuition rates for the 2021-2022 school year will be posted on the LILA website, www.MyLILA.org, in January 2021.

You will need the following information to complete this form:
- Names and phone numbers for two local emergency contacts that will be notified in the event we are unable to reach you. These persons must also have permission to pick up your child from school.
- Names and contact information for health and dental care providers, and your preferred hospital.
