BRK Waiver:
I agree and give permission for the The Big Rig Kids Organization, SE&E Co, and KJ Multimedia Solutions (Production Companies) their staff, affiliates, and volunteers, to videotape or photograph myself and record my voice and likeness for the purpose of the production mentioned above, whether by video recording(s), motion picture filming photograph(s), audio recording(s) or otherwise;
I authorize the use of any such electronic use of my name and likeness for the purposes of education, promotion or advertising, including the sale or trading of the recordings and any copies so made for any purpose, including, but not limited to education and other public media, as may be deemed appropriate (I understand that I may be identifiable from such electronic reproductions).
I hereby authorize the “Production Companies” and their affiliates to copyright, publish, broadcast, digitize, display and otherwise use (a) my name, image, likeness, and voice, and (b) all recordings, videotapes, audiovisual materials, writings, statements, and quotations of or by myself (collectively, the “Materials”), in any manner, form, or format whatsoever now or hereinafter created, including on the Internet, and for any purpose, including, but not limited to, advertising or promotion of Companies, its affiliates, or their services, without further consent from or payment to me for “the program” expressed above.
It is understood that all of the Materials, and all films, audiotapes, videotapes, electronic and digital copies of the Materials recorded by the “Production Companies”, are the sole property of the “Production Companies”. I agree not to contest the rights or authority granted to Companies hereunder. I hereby release and discharge “Production Companies”, its employees, licensees, agents, successors, and assigns from any claims, actions, damages, liabilities, costs, or demands whatsoever, arising by reason of defamation, invasion of privacy, right of publicity, copyright infringement, or any other personal or property rights from or related to any use of the Materials. I understand that the Companies are under no obligation to use the Materials.
I understand and agree that I will not receive compensation of any kind in connection with any use of any such electronic reproductions or any other rights granted by me hereunder, and that the producer(s) and their assigns, are relying on the rights granted herein.
I also represent that I am eighteen (18) years of age or older (or if I am under eighteen (18) years of age my legal guardian has read, and signed below, in order to consent to all the foregoing).
I further understand the master tape(s) and (digital recording(s)) remain the property of the Producers and there will be no restrictions on the number of times that my name and likeness may be used. *
