PB Theatricals, HMS Pinafore - Child Registration Form
This form is for parents/guardians of children aged under 18 who wish to register to perform in HMS Pinafore.

Sections 1, 2 and 4 are to be filled in by the parent/guardian, section 3 by the child.

If you wish to register more than one child please return to our website once you have completed this form and fill out another (one form per child).

If you are over the age of 18 and wish to be in the cast of our young adult show Princess Ida, please return to our website and select the other form.

Important information about rehearsal and performance dates/times
Auditions are at 10am on Saturday 3rd August at the Playhouse, Chapel-en-le-Frith
Rehearsals from 12th - 17th August at the Medway Centre, Bakewell
Evening performance on Saturday 17th August at the Medway Centre;
Matinee performance on Sunday 18th August at the Playhouse, Chapel-en-le-Frith.

Rehearsals typically take place between 10am–5pm every day during rehearsal week. We will send out more information in our registration pack in the first week of July, but if you have any questions please don't hesitate to get in touch with us at pbtyouththeatre@gmail.com.

PB Theatricals is a not-for-profit organisation, however, we charge a fee per cast member in order to cover our production expenses and allow us to continue to offer theatrical experiences to young people.

The early-bird registration fee is £50 and registrations after 1st July will cost £60. 

How will you be paying this fee?

NB - We recognise that not everyone will be able to pay this fee and we have some bursary places available, as well as flexible payment options. If you wish to discuss these options, please select option 3.

Section 2: Medical Questions (to be filled in by the Parent/Guardian)
The following questions are required to ensure that we can keep your child safe during rehearsals in case they have a medical emergency.

I give PB Theatricals Youth Theatre permission to take photos/videos of rehearsals and performances which may be shared publicly on social media for the purposes of publicising/recording this year's show and PB Theatricals Youth Theatre shows in the future (you reserve the right to withdraw permission at any time). *
Please ensure your child has read and understood the following. By clicking the "Submit" button you are agreeing to the following conditions;
- I agree to attend all rehearsals and performances when required (unless otherwise agreed with the Production Team far in advance) between the 12th–18th August 2024 (Rehearsals are typically between 10:00am–5pm every day during rehearsal week. A more detailed rehearsal schedule will be provided before rehearsals begin);

- I agree to attend the auditions on 3rd August 2024 even if I wish to be in the chorus;

- I agree to do my best to listen to the music of HMS Pinafore before rehearsals begin so I am already familiar with the show before rehearsals begin;

- If I cannot attend any rehearsal/performance for any reason including illness, I will let the Production Team know as soon as possible by emailing pbtyouththeatre@gmail.com;

- I agree to be polite, attentive and respectful to the Production Team during rehearsals;

- I agree to contribute to a positive environment within PB Theatricals Youth Theatre and I will not intimidate, bully or disrespect any other cast members or production team members;

- When asked by the Production Team, I agree to supply any pieces of costume or, if I am unable to, inform the Costume Designer as soon as possible so other arrangements can be made (a detailed list of costume will be provided before the show);

- I will contact members of the Safeguarding Team if I have any concerns over the safeguarding of any cast members under the age of 18;

- I agree to not attend official PB Theatricals rehearsals/performances under the influence of drink/drugs and I understand that any attempt to supply said substances to cast members below the age of 18 will result in the police being contacted and expulsion from the show;

- I understand that any violation of the above may result in being expelled from the show.
