The 2021 Holiday Celebration and Silent Auction will take place on November 17, 2021, at The Fillmore Minneapolis event venue, and will run from 3:00 to 6:00 p.m.

Please provide information about the donor and the item in the form below.
* You will receive a copy of your completed form which will include instructions for getting the donated item(s) to our office.
* The person listed below will also receive documentation to provide to a tax advisor to determine if it is tax-deductible.
* If you are submitting this form for a basket that contains items from multiple donors, we ask that you fill out this form for each separate item (so that staff can provide tax documentation to each donor for each item).

The deadline for completing this form has been extended to Monday, October 25, 2021.

Items must arrive at the Chapter office by Friday, October 29, 2021.
