Fundraising Request Form 2021-2022 School Year
This form must be completed in full and submitted at least two (2) weeks prior to the requested fundraising activity.
All request forms must be submitted by either a Head Coach, Moderator or their one (1) authorized representative.
Parents and players must be notified of the activity they are requested to participate in at least two (2) weeks prior in advance of the fundraising activity.
Students/families cannot be required to sell/purchase items.
