Please answer all of the following prompts to allow your child to participate in the JCPS Driver Education Program.  This information is to obtain a permit that is valid only for instruction by a CERTIFIED DRIVER EDUCATION TEACHER in a DRIVER ED CAR for students under the age of 18. The permit expires at End of Roadwork Session.

(Please make sure you have a current meal application on file with the School Nutrition office. You can complete one at https://family.titank12.com.) *
