Students may opt-OUT of live Zoom specialist instruction and complete independent enrichment projects for course credit. The purpose of this change is to flexibly address student/family feedback regarding screen time, while continuing to offer meaningful enrichment programming to students. Students may ONLY opt-out of live instruction with parent permission granted through this form EACH month.

Parents/guardians may also use this form to enroll student(s) in grades 3-6 in small group instrument lessons!

Parents/guardians ONLY may submit this form on behalf of their child/children each month.
Form submissions made using student email addresses will not be accepted.
The form will be posted in the “specialists” tab of our Home Learning Site and emailed to parents/guardians.
All assignments will be posted on SeeSaw and Google Classroom (depending on your child’s current platform).
To receive credit, students must post their work by the assigned due date.
Due to the scheduling of projects and in-person units, students who opt-out for the month cannot switch back into or attend live classes until the following month to prevent disruptions to the learning environment and to maintain instructional consistency for both groups of students.
