RELEASE FORM

I hereby release the Nelson County Fairboard and any associated companies,
persons, or organizations of any liability. I authorize the Nelson County Fairboard to
use photos, names, etc. for the publicity and news reporting, social media, taped
interviews, and on the spot broadcasts as the fair board sees fit.

I understand that if my child is chosen as Miss Teen Nelson County Fair, I will help my
child represent Nelson County to the best of our ability. If for any reason the winner
cannot attend the state pageant or the opening ceremonies of the Kentucky State
Fair the first runner-up will take her place.

Good sportsmanship is required by the contestant and their guest and family members. I understand that my child will be disqualified if poor sportsmanship is displayed.

I am allowing my child to enter this pageant with the full understanding of the rules
and regulations and accept the fact that the judges’ decision is final and
incontestable.

I have read and understand that by signing below am giving permission for my
child to enter this pageant.
