Dear Parent/Guardian,

In line with the Work Safely Protocol, the key to maintaining a safe school requires strong communication and a shared collaborative approach between the BOM/ETB, staff, students and parents.

The assistance and cooperation of all staff, students, parents, contractors and visitors is critical to the success of keeping us safe and well.

This form must be used when children are returning to the school setting after any period of absence and should be updated accordingly.

It is compulsory for all parents/guardians or students over 18 to complete this form prior to their son/daughter returning to school, any child returning without a completed form may be refused entry.
