Due to program capacity, completion of this form does not mean your child has been accepted to the Suns Youth Basketball Program. Priority will be given to returning players and new players will be entered into a waitlist. Completion of this form will automatically enter your child into the waitlist. We will contact you if your child is selected for the program.

EXPECTATIONS OF PARENTS AND PLAYERS
As members of The Philadelphia Suns, my child and I are expected to behave in a manner that is consistent with our team philosophy. My child and I agree to place the highest value on hard work, teamwork, commitment, respect, honesty, integrity, and to have the proper attitude and perspective in everything that we do to build community.
By completing this form below, I give permission for my child to participate in The Philadelphia Suns’ efforts to build and support a better community. I understand that my child is required to participate in team functions which may include attendance/support at: community service projects, New Year’s performances, team practices, tournament/league games, fundraising, and other wholesome social activities.
ATHLETIC/VOLUNTEER PARTICIPATION WAIVER, CONSENT & RELEASE
By completing this form, I hereby consent my child's participation in the Suns Youth Basketball Program sponsored by The Philadelphia Suns Inc. Furthermore, I hereby acknowledge that there are inherent risks associated and accompanied with the above stated sport/activity and that the child named may be injured as a result of an accident arising out of participation in the named activity. In consideration for permitting the individual named to participate in community building, through service opportunities and sports such as basketball, lion dance, and volleyball, the undersigned releases and holds harmless The Philadelphia Suns Inc. and /or its employees, teachers, coaches, administrators, volunteers et al., from any and all liability including, but not limited to, liability for injuries or damages sustained by the individual.
PICTURE RELEASE
By completing this form, I hereby authorize usage of pictures and videos of the individual named for any lawful promotional material of The Philadelphia Suns without payment or other consideration.
