I understand and agree to the following - (1) I will display the car tag either on the right side of the dashboard or on the passenger side window. (2) Only the adults I've listed will pick up my child, unless a note has been sent to school in the morning stating the name and relationship of the individual. Summitview WILL NOT release a child to an adult not listed on a child's emergency card or note signed by parent/guardian. (3) I will remain in my vehicle during pick up or will wait in the designated area, holding the car tag number. (4) My family will receive two copies of the number tag. *
