Welcome! We are so glad that you have decided to be part of CHOSEN for the year 2020-21. Please fill out this form to make it official!

A couple of things to note:

- You are not finished filling out this online form until you click SUBMIT after entering grade/age information. You will see a confirmation page that starts with, "Thank you for registering for this year."

- Your membership will be finalized when we receive your membership payment either by check or Paypal.

- Your membership will be good for one year and will need to be renewed annually. Even if you are a renewing member, you need to re-enter all the information. It does not carry over from previous years.

- We are a volunteer-run organization (most of us are teachers of many children) so please allow up to one week for us to get back to you if you have questions.

- Please email the Membership Coordinator at membership@nwchosen.org if you have any questions.

How Can I Help?

We are blessed with many gifted and talented members. Sharing your gifts is a wonderful way to get to know our members better as well as offering support and more options for our children. Please check any way you would like to serve within our group (check as many as apply or add your preference to "Other"). Many hands make light work!
