The By Degrees Foundation is seeking nominations for the first annual Middleton Award for Educator Excellence. The Middleton Award will be presented to an educator who has made a significant positive impact in the lives of children through their role in a school or classroom. Educators working in any of the schools in which By Degrees provides programing (currently Findley Elementary, Harding Middle, and North High) may be nominated. 
In addition to the following information, you may submit up to two pages of additional documentation in support of the nomination. Additional materials may include letters of support, photos, additional information on their work, etc.
