This form is to be completed because a student is requesting to take a recovery course. That means that the student took the course previously but did not pass the course and needs the course credit for graduation requirements. This form is NOT for a student who needs an initial credit.

Please note that credit recovery classes are NOT approved NCAA courses. Also, please note that some courses may require an EOC to receive a final grade.

It is imperative that you speak with your counselor prior to completing this form. No request will be honored if the student has not spoken with the counselor first.

Requests will be granted based on Cohort and need. Thank you for your patience.
