Description: 
  1.  4-H Junior Achievement Awards are designed to recognize the outstanding 4-H accomplishments of Juniors (8-11 years of age). Age is of January 1 of the current year.  
  2. Nominees for Junior Achievement Awards must have completed more than three years in 4-H and not qualify for the Junior Incentive Award.
  3. Members can only win this award once.  
  4. This form should be completed by the 4-H nominee.
