Hi,
We would like you to fill in this form three times a year* if you are coordinating a CBS class.  
If you are leading a church group we invite you to support the ministry when you start a new study.  This form can be used by either type of group.  
Start by entering your own email address and when you finish the form we will send you an acknowledgement email.
If you need to return to edit the form you can do that from a link included in that email.

Anyone under the age of 18 counts as a child.
CBS or Church-led group? *
Some groups have their own CBS leadership are typically interdenominational and others are led by a church team.  We support both types of group and they have equal access to materials but may choose to relate to CBS in different ways especially financially.
