By submitting this registration form, I understand and agree to the following statements:

1. I will be required to pay a $30 non-refundable registration fee for each child attending.
2. I will be required to pay a 50% deposit of my child's tuition at time of registration to hold my child's spot.
3. I have read and agree to the Summer Enrichment Academy Policies located on the FUNdamental Minds website. I understand a signed copy is required before my child is admitted into the program.
