By signing up for the event, I hereby consent to the use of my photograph, voice and image in any broadcasts of this event and in subsequent productions drawn from video or audio recordings of this event. The photographs and recordings may be published or broadcasted in public publications and in publicity materials, including SFCCA’s websites and social media. 宗乡总会将在活动期间进行摄影和/或录影，所拍摄的影像将有可能充作宣传及推广用途。

3) I am authorised to collect and submit the above information on behalf of the individuals listed above for the purposes of participating in the activity. I declare that they are aware of the information the SFCCA holds about them.
