Final Transcript Request Form
Any senior who is attending college must complete this Final Transcript Form. This form is due no later than May 12, 2023. The one college or university that you plan to attend in the fall needs your final transcript to verify that you graduated so that you can attend their college. Your final transcript will be mailed by our counseling secretary on or about June 23, 2023.
