Clinical Lactation Training Program Application Form
BFLA's Clinical Lactation Training Program is designed to offer graduates of the 95 hour Lactation Consultant Education course a chance to receive hands-on experience in clinical lactation management. Once admitted to the program, students will be paired with an experienced IBCLC mentor at local healthcare facilities in Los Angeles. During placement, students will have the opportunity to gain exposure to and experience with the clinical competencies set forth by IBLCE, mastering skills and gaining the hands-on experience required to sit for the IBCLC exam.

Program Eligibility & Requirements
Application requirements include:

1. Application fee of $15
2. Commitment to complete 500-1000 hours (depending on pathway) of clinical internship over the course of the program
3. Proof of completion of 95 hours of education in human lactation and breast/chestfeeding within the last 5 years
4. Resume/CV
5. Two (2) Letters of Recommendation
6. Interview with BFLA Clinical Director & Clinical Placement Committee

Required prior to clinical placement (details given upon acceptance):

1. Health screening/clearance
2. Criminal background check
3. OIG Sanction Trace
4. Proof of malpractice insurance coverage
5. Seasonal flu shot/declaration of religious exemption
6. Proof of TB screening
7. Immunization Records (including MMR, varicella, Hep B, TDaP)
8. Pathway pre-approval from IBLCE
9. Completion of HIPAA training, Universal Precautions & Infection Control, Medical Documentation, Medical Terminology, Professional Ethics, Basic Life Support (BLS)


Application Process
Application packages consist of this online form, a resume, two (2) letters of recommendation and proof of completion of a 95 hour didactic lactation education program (a certificate of completion will suffice).  

Application packages are accepted on a rolling basis and reviewed monthly on or around the first of the month. Applicants will be contacted to schedule an interview shortly after each application review period.

Note: All items MUST be submitted before being considered. Incomplete applications will NOT be reviewed.

1) Answer questions in Section 2 of this application.

2) Upload resume, two (2) letters of recommendation, proof of completion of a 95 hour didactic lactation education program to Section 3 of this application as a PDF or Word document.

3) Submit!

Selected applicants will be invited to an in-person or video conference interview with the Clinical Placement Committee.
