THIS FORM MAY ONLY BE FILLED OUT BY PARENT OR LEGAL GUARDIAN. If NOT a parent or legal guardian, the registration may become VOID and participation may be withheld.
FEES: $50 to be paid at your student's home school or the David Douglas District Office.
PLEASE DO NOT PAY UNTIL YOUR PLAYER HAS MADE THE TEAM AFTER TRYOUTS
REFUND POLICY: Refund request must be made in writing no later than 30 days from the first scheduled game. Full refunds will be only issued if the student has not played in a game. $10 will be deducted for materials, handling, and uniform.
INSURANCE: Players need to either purchase school insurance or have family insurance. School insurance may be purchased at a minimal fee from your school's office. Medical insurance is a requirement for participation and is the responsibility of the parent or guardian.
JERSEYS: Players will be issued jerseys upon payment of registration fee. Players will RETURN their jersey immediately after the last game of the season. (Please bring extra shirt to wear home instead of jersey.) Lost or unreturned jerseys will be charged a $40 replacement fee.
