1. I have known the risks, consequences, benefits, or alternatives events as indicated above. I hereby declare that I give my consent to the event (s) with full knowledge and capacity and acknowledge that the Event organisers provide no warranties, regarding my wellbeing and safety.
I certify that I am physically fit and/or have not been advised to not participant by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activities or events.

2. Terms of Media Consent:
I give permission for ACET to: use my image and/or voice in photography, film and sound recordings in promotional materials for ACET’s goods and services as well as activities supported by ACET; Edit/format my image and/or voice as necessary for the type of media used for the promotional materials.
I understand that:
the promotional materials may be in any media, as per the approved platforms selected above and the use of my image and/or voice in these may extend to communications and marketing materials for ACET
and related third parties (as determined by ACET); and
All recordings of my image and/or voice taken by ACET will be owned by NEDF; and I have the right to request ACET to stop using my image and/or voice at any time.
3.General Data Protection Regulation (GDPR)
At ACET we take your privacy seriously. Please confirm you consent to us obtaining, processing and storing your information. Please note we will not share your personal information with anyone without your consent.
