Any time you read or listen to a book this summer, complete and submit this form to be entered into a drawing for a $25 gift certificate to a local restaurant!
Your first five entries will be entered into the main raffle. Each additional entry (over 5 books) will count as an entry for a bonus raffle. One entry per person for the Grand Prize Raffle. One win per person in the Bonus Raffle Drawings.
