All children under 18-years-old are now required to provide parent contact information in order to attend all Parkallen Community League programs at the hall and park. Parents are responsible for their children at all times and must be available in the event of an emergency. If this form is not completed and a valid parental contact number is not provided, then your child will not be allowed to attend.

Please note that this is for the safety of your child. If your child is injured, has a medical episode, or is acting inappropriately while attending Parkallen programming, you will be asked to pick them up immediately. Parents must be accessible for pickup at any time during the event/program.
