I understand that this form must be filled out in its entirety, and once I submit this form, the card I have placed on file for my reservation will be charged for 50% of my final transportation cost (unless I have a package that includes transportation). I also understand that any changes or cancellations to my requested transportation must be made at least 72 hours in advance via email, and failure to cancel within 72 hours of the scheduled transportation will result in my 50% payment being forfeit. *
