LASA Class of 2021 Senior Salute and Project Graduation Release of Liability/Non-alcohol and Drug Use Agreement
PLEASE READ THIS DOCUMENT CAREFULLY.  Returned forms should have a signature and contact information  of both student and at least one parent. Students must be on the official LASA graduation list from the Registrar’s office to attend.  Completed forms are required for admittance.  

Senior Salute/Project Graduation Policies
• Students must arrive for Senior Salute between 5:10 and 5:40 PM.  The event starts promptly at 6:00 PM.  Students may leave after Senior Salute but cannot then come back for Project Graduation.
• Project Graduation event hours are 8:00 - 11 PM. Students who did not attend Senior Salute may come to Project Graduation but must arrive prior to 8:45 PM.
• The event and all activities and food are FREE for LASA HS graduating seniors.
• Temperature checks will be taken and the door and masks are required at all times.
• This is NOT a lock-in event. Attendees who leave the venue (for any reason) will NOT be readmitted.
• This is a drug- and alcohol-free event. Any student caught bringing illegal substances into the event will be detained and parents notified.
• Students agree that they will not engage in drug or alcohol use for the remainder of the night after leaving the event.
• Transportation will NOT be provided but there is plenty of venue parking.
• AISD clear bag policy is in effect for this event (no backpacks, bags/change of clothes allowed). Cell phones/cameras will be allowed but are the responsibility of the student.
• Students must bring a picture ID (Driver’s License or LASA ID) to check-in the night of the event.

(Student name in question above) plans to attend Senior Salute and/or Project Graduation (hereafter know as “Event”) at Springdale Station on June 2, 2021.  We acknowledge that this is a drug-free and alcohol-free event and agree to abstain before and after this event.  (Student name in question above)  agrees to abide by the policies set forth for the Event by the following defined Entities and individuals acting on their behalf: LASA Senior Salute and Project Graduation 2021 and volunteers, Austin Independent School District, and LASA High School (herein referred to as the “Entities”).   We also grant to the same Entities and to individuals acting on any of the Entities’ behalf permission to act as the student’s agent according to the individuals’ best judgment in an emergency situation during the Event. We agree to indemnify and hold harmless such Entities and individuals acting on any of the Entities’ behalf from all claims for loss, damage or injury during the event and during any emergency situation that arises.   Both the parent and student must click the two boxes below: *
