I, the undersigned, am the parent/guardian of (name of student above)and give my authorization for him/her to participate in the trip operated by World Expeditions of San Diego, and acknowledge and agree to the following provisions:Trip Supervision: The primary responsibility of all adult chaperones will be to ensure the safety and well-being of students.All duly-authorized chaperones, while on this trip are acting as agents solely for World Expeditions, and not for any other entity, school or district. In the absence of a student’s parent or guardian, chaperones will have full supervisory responsibility and authority for all student minor participants as long as the student is a participant in this trip.Limits of Supervision: All adult chaperones have a responsibility to supervise multiple students on the trip. We cannot designate a chaperone as a full-time supervisor for just one student. A chaperone will always be able to take a student for necessary medical treatment and wait a reasonable amount of time for treatment to be completed. If, in the unlikely event that a student participant becomes unable to continue to travel due to illness, hospitalization, incarceration or other reason that prevents them from continuing on the trip as scheduled, the trip chaperones will no longer have responsibility for supervising that student, and the student may be left in the supervision of local health or law enforcement authorities. The parent or guardian will need to immediately travel to the student’s location at their own expense and assume responsibility for their child.  Behavioral Release: World Expeditions has the right to decline to accept or retain any person, adult or minor, as a member of a tour at any time for any reason before or during the actual tour. Student use or possession of alcohol or drugs,possession of weapons, repeated insubordination, self-endangerment, harassment or other similar behavior that endangers their safety, the safety of another person, or causes a significant adverse effect on the quality of the trip for the other participants, may be cause to remove a student from the trip, and for chaperones to seek treatment or remand them to local authorities. At that point the student is no longer a participant in the trip, and a parent or guardian will be responsible for taking custody of their child. Additionally, any damage caused by a participant will be the financial responsibility of the participant or their parents.Medical Incapacitation: Students who are admitted for hospitalization due to physical, emotional or psychological issues may not be capable of continuing on the tour as scheduled. In such a case, the student will be left under the supervision of the medical facility. The student will no longer be a participant in the trip, and a parent or guardian will need to immediately travel to the student’s location at their own expense and assume responsibility for their child.Trip Activities: This is an educational-enrichment trip and students will be expected to be actively engaged in learning at all sites visited. They must also adhere to rules similar to those at a school and any other rules given by chaperones. Students who fail to do so may be disqualified and returned home at parent’s expense at any time.Travel Delay, Cancellation or Interruption: In the event of an unforeseen delay, cancellation or interruption of travel services during the trip, any additional expenses that occur will be the responsibility of the individual participant or their parent. In most cases, World Expeditions will advance payment for participants as a group, and will invoice participants or their parents for reimbursement. In some cases, participants may seek reimbursement from the included trip insurance coverage and/or specific vendors as appropriate.General Release: World Expeditions’ responsibility is strictly limited. World Expeditions and/or its representatives act as agents for the tour participants in all matters connected with hotel accommodations, sight-seeing, restaurants and transportation and as an agency holds itself free of responsibility for any liability occurring due to the performance of the contractors supplying services for this tour. Neither World Expeditions nor its agents or employees shall be liable for any accident, injury, illness, death, property damage or personal loss, expense or inconvenience in connection with any accommodation, transportation or travel services that result directly or indirectly from any occurrence or conditions beyond their control including but not limited to, defects in vehicles, breakdown in equipment, theft, delay or cancellation, late departures, change of schedule, strikes, acts of God, commercial frustration, acts of war or terrorism, government or industry shutdown, or the willful or negligent act by participants, or by the willful or negligent act or omission by the provider of services or their respective representatives including without limitation, their failure to deliver, or inadequate delivery of travel services.  Executed in the County of San Diego, State of California on (type date here) *
