The purpose of this document is to inform me about the rules and risk about online video peer based support groups conducted by Tynysha Lige, LSW Asiah Lemon, MPA and Nathasha Nazaire, LPC. I am aware that this is not a counselling session, this is an online support group with the purpose of sharing information and resources. Information shared or expressed by my youth, will be kept confidential. In addition, I am aware of the risks of online support groups such as technology disruption, and transmission of information that can be disrupted by an unauthorized person. Please note that confidentiality will be breached if there is reason to believe any form of abuse, self harm, suicide, and intent to harm someone. I acknowledge I have read the above statement and consent to my child participating in this online support group. *
