TRANSPORTATION INFORMATION
I understand that my child must ride the bus to Minute Maid park with the CMS Music Department. *

My child will ride the bus back to CMS from Minute Maid Park, leaving seats in the stadium at 9:15 p.m. *
I will check my child out from their music teacher at 9:00 p.m., staying for a longer time at the game, and provide their transportation home from Minute Maid Park. *

RULES & PROCEDURES INFORMATION
I understand that my child must have their part to "The Star-Spangled Banner" memorized by August 23 to perform at the game as music stands are not permitted on the field. *

I understand that my child must attend the two rehearsals before school (starting at 9:15 a.m.) on August 18 and 25 at CMS in order to perform at the game. *

I understand that if my child doesn't meet one or both of the requirements noted above, they will still attend the game with our group but cannot perform on the field. *

I understand that my child must wear the white t-shirt provided for this performance, jeans (in dress code), and closed-toe shoes. *

I understand that my child will be seated with their musical organization during the game, unless I check them out at 9:00 p.m. *

I understand that my child will be placed in a group with other students to be supervised my an assigned adult chaperone during the game. *

I understand that my child will need to bring funds (no cash) via debit or credit card to purchase food and drink during the game. *
