I do hereby give my son/daughter, permission to be a participant in Community Technical Assistance, Inc (CTA), Global Scholars Program. I understand that my son/daughter may be included in photos/videos during the course of their participation with CTA and I hereby give my permission that their likeness may be used in print or video for sole use by CTA. *
