We ask that all clients complete a registration form, even if you are current student and are not anticipating any changes to your ongoing services. For scheduling purposes, it is also necessary for each student to submit an individual registration.

Many factors go into reserving time spots just for you; the biggest factor is the date and time stamp on your completed registration. Please note, your completed registration form does not guarantee a preferred spot. We will do our best to align your preferences to a spot assignment.
