After School Study Group Student Request/Permission Form
Please complete this form if your student will be in attendance for After School Study Groups (ASSG).
ASSG will meet every Tuesday and Thursday in the high school library from 2:40 until 4:15.
A maximum of 25 students per day will be admitted to the library.
Two to three teachers will be present and assist the students with their work.
Students in grades 7-8 must have this form completed and submitted no later than 2:35 the day of the meeting.
Students must work on school material. No playing games or watching videos for entertainment will be tolerated.
TRANSPORTATION WILL NOT BE PROVIDED.
